Abstract

Introduction: Preeclampsia (PE) is a complication of pregnancy, burdened with heavy maternal-fetal morbidity and mortality. It complicates 2–8% of pregnancies worldwide [1–3]. In addition to the clinical and biological monitoring of mother and child, doppler ultrasound is an essential tool and makes it possible to assess the risks for the fetus and to guide management [4]. The aim of this work was to study the place of Doppler ultrasound in the management of patients admitted for PE in the 3rd trimester of pregnancy in order to contribute to the reduction of maternal and perinatal morbidity and mortality of this pathology.

Materials and Methods: Descriptive and analytical cross-sectional study with retrospective collection over a period of 25 months from September 1, 2019 to September 30, 2021. It included patients admitted for PE whose gestational age at the time of the realization of the obstetrical Doppler ultrasound was higher than 28 SA and having given birth at the maternity hospital of the CHU of Bogodogo. For each patient we noted the age, the profession, the marital status, the gyneco-obstetrical history, the gestational age at the time of the ultrasound, the blood pressure, the proteinuria of the 24 hours, the results of the echo - obstetric doppler, mode of delivery, maternal complications, fetal and neonatal complications and birth weight. Data were analyzed using Epi info 7.2.4.0 software. Comparison tests were performed using the Chi 2 test and Fisher's exact test with a statistical significance level of 5% (p < 0.05).

Résults: Average age of pregnant women was 30.24 years. Patients over 35 years old represented 29.34% of our sample. Housewives represented 53.26% and those living in a marital relationship 80.44%. The average number of pregnancies was 3.06 and 33.70% of patients were nulliparous. A history of in-utero fetal death was noted in 7.60% of them. At the clinic, 57.61% of patients had severe hypertension. On Doppler ultrasound, the average gestational age was 34.78 weeks, the cerebral resistance index was low for 36.08% of the fetuses and the umbilical resistance index was high in 49.48% of the cases. The cerebroplacental index (CPI) was less than 1 in 40.21% of the fetuses. Severe hypertension was a risk factor for pathological Doppler. Caesarean section was the most frequent mode of delivery (70.65%). We recorded 12.37% stillbirths, 59.76% premature, 51.81% low birth weight among live newborns, 34.11% transfers to neonatology and 9.41% deaths. early neonates. There was a statistically significant association between the high resistance index of the umbilical artery, the low resistance index of the middle cerebral artery, the cerebro-placental index less than 1 on the one hand and the occurrence of complications. perinatal fetuses with a P-value of less than 5% and an odds ratio greater than 1 (p<0.05; OR > 1). There was also a statistically significant association between the severity of hypertension and Doppler abnormalities on the one hand and also an association between the existence of Doppler abnormalities and delivery by cesarean section.

Conclusion: In the 3rd trimester of pregnancy, the occurrence of disturbances of the umbilical and cerebral Dopplers as well as the ICP was an element of a prediction of poor perinatal prognosis. Obstetric Doppler would improve perinatal prognosis by early diagnosis of complications by providing information on the quality of exchanges between the fetus and the mother during PE, thus improving management.

RÉSUME

Introduction : La pré-éclampsie (PE) est une complication redoutable de la grossesse, grevée d’une lourde morbidité et mortalité materno-fœtale. Elle complique 2 à 8% des grossesses dans le monde [1–3]. Outre la surveillance clinique et biologique du couple mère-enfant, l’échographie doppler est un outil incontournable et permet d’évaluer les risques pour le foetus et de guider la prise en charge [4]. Le but de ce travail était d’étudier la place de l’échographie doppler dans la prise en charge des patientes admises pour PE au 3ème trimestre de grossesse afin de contribuer à la réduction de la morbi-mortalité maternelle et périnatale de cette pathologie.

Matériels et méthodes : Il s’est agi d’une étude transversale descriptive et analytique à collecte rétrospective sur une période de 25 mois allant du 1er septembre 2019 au 30 septembre 2021. Elle a inclu les patientes admises pour PE dont l’âge gestationnel au moment de la réalisation de l’échographie doppler obstétricale était supérieur à 28 SA et ayant accouché à la maternité du CHU de Bogodogo. Pour chaque patiente nous avons noté l’âge, la profession, le statut matrimonial, les antécédents gynéco-obstétricaux, l’âge gestationnel au moment de l’échographie, la pression artérielle, la protéinurie des 24 h, les résultats de l’écho-doppler obstétricale, le mode d’accouchement, les complications maternelles, les complications fœtales et néo-natales et le poids de naissance. Les données ont été analysées à l’aide du logiciel Epi info 7.2.4.0. Les tests de comparaison ont été faits à l’aide du test de Khi 2 et le test exact de Fisher avec un seuil de signification statistique de 5% (p < 0,05).

Résultats : L’âge moyen des gestantes était de 30,24 ans. Les patientes de plus de 35 ans représentaient 29,34% de notre échantillon.  Les femmes au foyer représentaient 53,26% et celles vivants maritalement 80,44%. Le nombre de grossesse moyen était de 3,06 et 33,70% des patientes étaient nullipares. Un antécédent de mort fœtale in-utéro a été noté chez 7,60% d’entre elles. A la clinique, 57,61% des patientes avaient une HTA sévère. A l’échographie doppler, l’âge gestationnel moyen était de 34,78 SA, l’index de résistance cérébral était bas pour 36,08% des fœtus et l’index de résistance ombilical était élevé dans 49,48% des cas. L’index cérébro-placentaire (ICP) était inférieur à 1 chez 40,21% des fœtus. L’HTA sévère était un facteur de risque de doppler pathologique. La césarienne était le mode d’accouchement le plus fréquent (70,65%). Nous avons enregistré 12,37% de mort-né, 59,76% de prématuré, 51,81% de faible poids de naissance parmi les nouveau-nés vivants, 34,11% de transferts en néonatologie et 9,41% de décès néonataux précoces. Il existait une association statistiquement significative entre l’index de résistance élevé de l’artère ombilicale, l’index de résistance bas de l’artère cérébrale moyenne, l’index cérébro-placentaire inférieur à 1 d’une part et la survenue de complications fœtales périnatales avec une P-value inférieur à 5% et un Odd ratio supérieur à 1 (p<0,05 ; OR > 1). Il existait également une association statistiquement significative entre la sévérité de l’HTA et les anomalies doppler d’une part et aussi une association entre l’existence d’anomalie doppler et l’accouchement par césarienne.

Conclusion : Au 3ème trimestre de la grossesse, la survenue de perturbations des dopplers ombilicaux, cérébraux ainsi que l’ICP a été un élément de prédiction de mauvais pronostic périnatal. Le doppler obstétrical permettrait d’améliorer le pronostic périnatal par un diagnostic précoce des complications en informant sur la qualité des échanges entre le fœtus et la mère au cours de la PE améliorant ainsi la prise en charge.
