Diverticule para-urétral de la femme

Abstract

Objective: To contribute to improving the management of patients with female paraurethral diverticulum.

Case report: Paraurethral diverticulum is a rare and benign condition characterized by the formation of a pouch near the urethra in women. Imaging studies can help in the diagnosis of this condition. We report the case of Mrs. MB, 33 years old, G2P2, with the last child aged 2 years, who presented 9 months after delivery with progressive urinary symptoms including dysuria, recurrent urinary tract infections and dyspareunia. The retrograde and voiding cystourethrography performed showed a subvesical image corresponding to the diverticulum. Complementary pelvic ultrasound was performed by suprapubic and endovaginal route followed by color and power Doppler studies which revealed a cystic mass located in the urethra region. The uro-CT performed allowed to objectify the diverticulum in the late phase in the form of a well-defined rounded formation of suburethral location. MRI could not be performed. The patient underwent transvaginal diverticulectomy. A urethrovesical catheter was left in place for 10 days. A follow-up examination after a few months showed no abnormalities.

Objectif : Contribuer à l'amélioration de la prise en charge des patients présentant un diverticule para-urétral féminin.

Rapport de cas

Le diverticule para-urétral est une affection rare et bénigne qui se caractérise par la formation d'une poche près de l'urètre chez les femmes. Les études d'imagerie peuvent aider au diagnostic de cette condition. Nous rapportons le cas de madame MB âgée de 33 ans, G2P2 avec le dernier enfant âgé de 2 ans qui a présenté 9 mois après son accouchement des symptômes urinaires progressifs comprenant une dysurie, des infections récurrentes des voies urinaires ainsi qu'une dyspareunie. L'urétrocystographie rétrograde et mictionnelle réalisé montrait une image sous vésicale correspondant au diverticule.  L’échographie pelvienne Doppler complémentaire a été réalisée par voie sus-pubienne et endovaginale avait permis de mettre en évidence une masse kystique située dans la région de l'urètre.  L’uroscanner réalisé a permis d’objectiver au temps tardif le diverticule sous forme d’une formation arrondie bien limité de siège sous urétrale. L’IRM n’a pas pu être réalisée. La patiente a subi une diverticulectomie Transvaginale. Un cathéter urétro-vésical a été laissé 10 jours. Un examen de suivi après quelques mois n'a montré aucune anomalie.
