Résumé

Objective: To describe anatomical variants of the renal and gonadal arteries using computed tomography (CT).

Materials and Methods: This descriptive cross-sectional study was conducted from May 5th to July 4th, 2024, in the CT scan units of two hospitals in Lomé and one in Kara. Data from patients who underwent abdominopelvic CT scans were collected using the Picture Archiving and Communication System (PACS). The study focused on the heights of origin of the renal and gonadal arteries, the presence of additional renal arteries, early bifurcation of the renal artery, its caliber, and the distance to the ipsilateral gonadal artery.

Results: Among the 124 patients included in the study, 71 (57.3%) were women (sex ratio = 0.74). The renal arteries were analyzed in all participants, while the gonadal arteries were assessed only in 115 patients due to their small caliber. The main renal artery emerged from the aorta between the upper border of the first lumbar vertebra (L1) and the lower border of the second lumbar vertebra (L2): on the right side in 98.4% of patients and on the left side in 97.6%. The renal artery was located below the superior mesenteric artery, at an average distance of 8.6 ± 6.3 mm on the right and 10.1 ± 6.7 mm on the left. Each kidney was supplied by a single renal artery in 66.1% of cases, while 33.9% had an additional renal artery. Additionally, 24 participants (19.4%) exhibited early bifurcation of the renal arteries. The average caliber of the renal arteries was 6 ± 1.1 mm on the right and 6.2 ± 1 mm on the left. Gonadal arteries primarily originated at the level of L2 (51.6% on the right and 50.8% on the left), located on average 27.4 ± 12.6 mm below the right renal artery and 27.4 ± 12.5 mm below the left renal artery. A right gonadal artery originated from an additional renal artery.

Conclusion: The presence of anatomical variations of both renal and gonadal arteries highlights the importance of detailed CT angiography scanning evaluation to optimize surgical interventions in the retroperitoneal region.

RÉSUMÉ 

Objectif : Décrire à la tomodensitométrie les variantes anatomiques des artères rénales et gonadiques chez les sujets adultes au Togo.

Matériels et méthodes : Étude transversale descriptive avec collecte prospective des données des patients ayant réalisé une tomodensitométrie de la région abdomino-pelvienne. Elle s’est déroulée entre le 05 mai et le 04 juillet 2024 dans trois établissements hospitaliers dont deux à Lomé et un à Kara. La hauteur de naissance des artères rénales et gonadiques, la présence d’une artère rénale additionnelle, d’une bifurcation précoce de l’artère rénale, du calibre et de la distance séparant l’artère rénale de l’artère gonadique homolatérale ont été déterminés chez chaque participant.

Résultats : Parmi les 124 patients inclus dans l’étude, 71 (57,3%) étaient des femmes (sex-ratio=0,74). Les artères rénales ont été analysées chez tous les participants, tandis que les artères gonadiques ne l’ont été que chez 115 patients en raison de leur finesse. L’artère rénale principale émergeait de l'aorte entre le bord supérieur de la première vertèbre lombaire (L1) et le bord inférieur de la deuxième vertèbre lombaire (L2) : à droite, chez 98,4% des patients et à gauche, chez 97,6%. L'artère rénale naissait en dessous de l'artère mésentérique supérieure, en moyenne à 8,6 ± 6,3 mm à droite et à 10,1 ± 6,7 mm à gauche. Chaque rein était vascularisé par une seule artère rénale dans 66,1% des cas, tandis que 33,9% présentaient une artère rénale additionnelle. Par ailleurs, 24 participants (19,4%) ont présenté une bifurcation précoce des artères rénales. Le calibre moyen des artères rénales était de 6 ± 1,1 mm à droite et 6,2 ± 1 mm à gauche. Les artères gonadiques naissaient majoritairement de l’aorte au niveau de L2 (51,6% à droite et 50,8% à gauche), situées en moyenne à 27,4 ± 12,6 mm sous l'artère rénale droite et à 27,4 ± 12,5 mm sous l'artère rénale gauche. Une artère gonadique droite naissait d’une artère rénale additionnelle.

Conclusion : La présence des variantes anatomiques des artères rénales et gonadiques souligne l'importance d'une évaluation angioscanographique détaillée pour optimiser les interventions chirurgicales dans la région rétropéritonéale.
